What it is
Infection of the coiled tube behind the testis — chlamydia or gonorrhoea in young men, gut bacteria in older men.
Symptoms & signs
- Painful swollen scrotum developing over a day or two
- tender lump behind the testis
- fever
- burning on urination or discharge
- pain eased by lifting the scrotum

- call 112 / 911 now Sudden severe testicle pain, especially with vomiting: could be torsion
- call 112 / 911 now High fever, shivering or confusion with a swollen scrotum
- see a doctor soon Swollen, tender testicle with discharge or burning on passing urine
- see a doctor soon No improvement after 3 days of antibiotics

Medical treatment
- Ultrasound or surgery to rule out torsion if sudden
- ceftriaxone plus doxycycline if an STI is likely
- ofloxacin or levofloxacin for gut bacteria
- scrotal support, ice and NSAIDs
