Medical treatment

  • Exercise therapy and weight loss — the best-supported treatment there is, relief comparable to NSAIDs trials support
  • Paracetamol barely beats placebo here; topical NSAIDs do better mixed evidence
  • Oral NSAIDs — real relief, real gut, kidney and heart risk when taken for months; rofecoxib (Vioxx) was withdrawn in 2004 over heart attacks its maker had played down mixed evidence
  • Steroid injection — weeks of relief, and repeated injections thin cartilage mixed evidence
  • Joint replacement once pain stops sleep and walking trials support
