Looks like

A V-shaped band of white matter deep in each hemisphere, between the thalamus and the basal ganglia.

What it does

A bottleneck where almost all fibres between the cortex and the brainstem and spinal cord pass, including movement and sensation.

Keeping it well

A small stroke here can paralyse the whole opposite side (face, arm, leg). Sudden one-sided weakness is an emergency.
